- 1 month old
- 2 months old (shots)
- 4 months old (shots)
- 6 months old (shots)
- 9 months old (lead & anemia screening. no shots given at this visit.)
- 12 months old (shots)
- 15 months old (shots)
- 18 months old (shots)
- 2 years old (shots, lead and anemia screens)
- 3 years old (vision screen, urine sample)
- 4 years old (vision screen, urine sample, we recommend getting your child’s kindergarten vaccines done at this visit)
- 5 years old (vision screen, urine sample, shots if not done earlier)
- Prior to 6th grade (shots)
- Prior to 9th grade (shots)
- Sports physicals are required annually.
We also recommend annual flu vaccines in the fall/winter (after 6 months of age).
