Vaccine Schedule
We use combination vaccines whenever possible, based on availability and insurance coverage.

| Birth: | Hepatitis B (1 shot) |
| 2, 4, & 6 MONTHS: | Dtap, Polio, Hepatitis B, HIB, Prevnar 13, Rotateq (3 shots, 1 oral vaccine) |
| 9 MONTHS: | No vaccines at this visit, Growth & Development, Lead and Anemia testing |
| 12 MONTHS: | Varivax & MMR/Measles Mumps & Rubella (2 shots) |
| 15 MONTHS: | Hepatitis A & Prevnar 13, (2 shots) |
| 18 MONTHS: | Dtap & HIB (2 shots) |
| 2 YEARS: | Hepatitis A (1 shot) Lead & Anemia testing |
| 3 YEARS: | Catch up any vaccines you may be missing, Growth & Development |
| 4 YEARS – 6 YEARS: | Dtap, Polio, MMR, Varivax (4 shots) |
| 6th Grade: | HPV, Tdap, & Menactra (3 shots) |
| 9th Grade: | as long as child is up to date on vaccines this should be a Physical Exam only. |
